Mike Gill

With over 40 years’ experience delivering transformation across NHS bodies, local authorities and commercial providers, Mike specialises in strategic change, bid management, service integration and organisational development, helping organisations achieve sustainable improvement in challenging circumstances.

Mike Gill brings deep insight into how improvement is achieved in complex environments. His career spans founding and building consultancy practices that have supported public and private sector clients over three decades, starting with several years in industry before working in a large local authority in Scotland. He founded Libre Consulting in 2000 (the previous name of At Scale) to sustain his passion for public sector improvement and translation of policy into operational reality.

Mike is driven by a deep commitment to helping organisations and teams deliver real operational improvement, particularly through collaborative service models and pragmatic, hands-on support. He works alongside clients to implement change rather than simply providing recommendations, supporting organisations to build internal capability whilst delivering immediate results. His approach bridges the gap between strategic aspiration and practical delivery, focusing always on what actually works.

Key achievements include:

  • Led the South East England-wide Sexual Assault Referral Centre procurement programme, coordinating NHS England commissioners, private sector providers, NHS Property Services, police, voluntary sector and service user voices to ensure continuity of sensitive, critical services throughout transition and support UKAS accreditation
  • Provided bid management expertise for over 300 successful public sector contracts across health, local authority, social care and specialist services, helping both public and commercial providers develop innovative, value-driven responses
  • Led tendering and mobilisation of urgent care contracts, bringing together acute trusts, ambulance services and GP federations, ensuring bids are financially sound, operationally deliverable and aligned with commissioner requirements
  • Delivered at-scale primary care programmes supporting practices bids and merger planning and implementation, working with PCNs on management structures, financial arrangements and business development strategies
  • Guided GP federations and practices through CQC special measures to achieve Good ratings, developing quality assurance frameworks and providing rapid response support for urgent improvement needs
  • Secured capital funding through NHS England and ICB approval processes for service development and premises expansion, building robust business cases and coordinating input from property experts and planners
  • Worked throughout the UK with health, local authorities and private sector clients on service transformation, competitive tendering, commissioning projects and service transitions and supporting implementation of new contracting arrangements

What distinguishes Mike’s work is his ability to translate between commercial and public sector cultures. He understands commercial imperatives around margin, risk and return on investment whilst also understanding public sector expectations around transparency, partnership working and social value. This enables him to help private providers position themselves effectively when engaging with NHS and local government commissioners.

Mike is author of “How to Survive Austerity: A Manager’s Guide to Doing More With Less and Emerging as a Leader in the New Public Sector”, presenting a practical framework for public sector managers facing budget pressures. He holds a BA (Honours) in Business Studies, is originally from Durham, and is now based in Edinburgh working throughout the UK.
