The Need

NELFT needed clearer insight into how young adults experience the transition from CAMHS to adult services and commissioned At Scale to shape a more engaging, responsive offer.

With rising concerns about transition quality, NELFT asked At Scale to review services for 18–25 year olds, focusing on engagement, continuity and support for young people from diverse communities.

What we did

We applied co-production principles, engaging young adults, families and staff, proactively reaching seldom-heard groups and reviewing transition processes to identify opportunities for more coordinated, timely and seamless care.

Outcomes

We delivered multiple recommendations across four themes, all adopted in principle. Our work reinforced the need for flexible, community-rooted, young adult focused services and showed how meaningful engagement strengthens design, giving the Trust evidence for service changes centred on lived experience.

At Scale’s approach ensured young adult voices genuinely shaped the findings. Their insights and recommendations give us a stronger foundation for improving transitions and designing services that reflect what young people say they need.
